I have integrated my form submission with google spreadsheets but only the first two fields of data are being entered to my spreadsheet. I tested this by changing the order of fields: seems that it doesn't matter in which order the fields are, only the first two get submitted.
What's this all about?

I checked your form and don't see any issue with it. I cloned your form and integrated it with google spreadsheet. I was not able to reproduce this issue.
Have you tried to removing the current google spreadsheet integration and adding it again? If not, request you to do that.
Let us know if you continue to have this issue and we will investigate further.

Ok I got it. I created the form first, then tested it for the first time and at this point it created the folders in google drive. Then I added more data fields.
Seems that you have to complete the whole form first and only then submit it for the first time as it creates the colums at this point and they can't be added later.
