Why my IP address is banned?

    Our organization just launched our form and we are running into this issue. We need support asap to fix this issue.

    Hello Ran!

    I would like to apologize for the inconvenience that you are experiencing.

    It seems that your account has been suspended for requesting banking information in the Void Check Upload field of your  PBHA Direct Deposit Form form. JotFrom does not allow the gathering of certain private information such as Driver's License information, Credit Card details, Banking Information, Uploads of ID's, etc.

    To have your account re-activated you will need to agree to our Terms of Use and allow us to delete the offending fields or the entire form. We will wait for your response to proceed.

    Additionally, if you would like to gather sensitive information such as this, you will need to do the following:

    1) Insert a Terms & Conditions widget into your form with your Terms of Use

    2) Insert an E-Signature widget into your form so your respondents can acknowledge that they are providing sensitive information

    3) Change the email address on your profile to one that shows your corporate domain (I can see you already have this).

    Please do let us know how you wish to proceed so that we may make the changes for you. We look forward to your response!

    So I have the E-Signature for the Direct Deposit. I just need the Terms &
    Conditions. Do you have suggested wording that is approved by jotform?
    Also, we would like to upgrade if it's more secure?

    Do you have suggested wording that is approved by JotForm?

    It is just an agreement that the user is sharing their sensitive information. You can find samples using Google Search. 

    We would like to upgrade if it's more secure?

    We have now deleted the offending fields in order to reactivate the account and added your IP address to our white-list.

    I’m a little confused! You can deleted it to reactivate our account.
    Are you saying that I can re-add the field as long as I have the terms and conditions?

    Yes, you can re-add them as long as you add the mandatory widgets first(terms and E-signature).

    I need some help! I'm struggling to find the Terms & Conditions on
    google. Since this is something jotform require, I think you should
    provide us with a standard format that's acceptable to you. Please advise.
