How can I create a restaurant menu with items (products) broken out on several pages?

Unfortunately, the payment widgets with ability to add products, forces them all together and then credit card info.

I would like to create a separate page for Appetizers, entrees, desserts etc.

Need the be able to calulate totals and feed into Authorize dot net widget.

Should be:

Description

Select yes/no

Price

Quantity

On as few lines as possible.

Yes, if you add products to your payment field it will display all on the same page and will require the payment information. However, if you would like to have products in your form display in different sections and then all calculate to a total amount you can certainly do that.

To do so, you will need to ass value to your product options and then use our Form Calculation widget to calculate a total and then pass that data down to your payment integration. Here are a couple of helpful user guides to assist you with doing this:
