Create mobile form to collect referral information

Hi 

I wanted to create a form to collect contact information ( references) from my customers. I want to collect name, telephone number, email address and address. I am expecting each customer to provide at 10 contacts. How can i design such form compatible to mobile ?

Please help if you have any resources or person to help me.

You can add a Configurable list widget to your form which will let you add multiple information dynamically.

Please build your form and if you feel like it is not responsive we will inject custom CSS to make it responsive.
