I was trying to integrate a jotform with google sheets, but it seems to only bring in the header info. I must be missing something.

I am assuming you're referring to this form: https://www.jotform.com/200794791448062
I checked the form and there's only one widget added to the form.
May we know what header are you referring to? Do you want to add the Tea Time Header in the Google Spreadsheet?

spreadsheet widget and made the table with 3 columns and 4 rows. Then moved
to Integrate and it created the spreadsheet on my Google Sheets with those
headings. Thanks!

Those were not headers. It is submission data to the Spreadsheet widget.
To create your first web form, please follow this guide: https://www.jotform.com/help/2-How-to-Create-Your-First-Web-Form
After that, you can add or integrate Google Spreadsheet to your form by following:
