I tested the form to make sure it worked, I filled out all the fields but they did not come across to the notification email.

Greetings and thank you for your message.
Please make sure that every field in the email notification has the corresponding field in the form.
To update your email, please go to the Settings tab, select Emails from the left menu and click on the edit button:
Then you can review your email. To change the field that is not displaying right, you need to remove the current field name:
And add the right one instead from the Form Fields dropdown:
If you click on the Employee Name, for example, you will see that {EmployeeName} has been inserted in the message. This text will later be replaced by the actual data.
Finally, don't forget to save the changes.
