Can you help me in real time to build the form?

Im wondering if you have someone who would help me in real time to fast-track  a comprehensive form for cafes to take online orders, in this difficult time. The ones you have are a start of course. 

We were going to set one up with our square account but it does not allow for orders to be paid at pickup, and the fees for online card swipes is really high, which means it's just going to be that much more of a money drain at at time when most of us can't afford that. 

I'd really love some dedicated help today and we will share it to as many local places as we can and it'll be a great addition to the template gallery!

Thank you kiran. I have had a negative experience with upwork which makes me leery of going that route again. I'll just find a simpler way to have our menu online for now and just have people call in orders. I wanted something I could share with my fellow cafe owners who suddenly can only do takeout because of the pandemic orders but i'm a little too unfocused to be able to figure this out on my own.
