Why is my account suspended?

    Good morning just wanted to know the reason for my form to no longer be found?

    I checked the email address through which you logged this thread and it is linked to a Guest account. If you have a regular account with us, please let us know the account username. Please also let us know the form URL that you are referring to so that we may trace the form if it's present in another account.

    So the account which I need it link to is office based and would be reshayw@relianceemployment.com currently I am unable to transfer the email to my personal devices. I am able to control the applications via the app on my phone which I have directed those unable to come to the office to register.

    I did check your account "reliance_reshay" and found that it is currently suspended. Your form "Simple Job Application Form" has ID upload and bank account related fields. 

    As far as IP upload field is concerned, you may collect identification, the numbers only, however it is a must to add an agreement displayed on the form and a signature, widget fields.
