PDF Form not being sent via template created

    I created a form that I need to have the results attached to an email confirmation. I customized the pdf for the attachment via the pdf designer editor tool and was sure to select the correct PDF when prompted but when I submit a test I get the other default pdf shown below.

    Hi,

    Please update your email advanced settings and enable the PDF attachment and select the PDF Document (NEW) then click the Save button.
