Account suspended

My account has been suspended and I need it back up right away.  I am an instructor approved by the Texas JBCC to teach process serving.  I have requirements that I have to meet when a student registers or takes a online class.  All my students information is required and secured.  I have students waiting to register and attend so please unlock my account right away.  Also, as part of my online webinar, I tell my students about your software when they are setting up their business, websites, etc.

The suspension was triggered due to the Driver License number, upload of Driver License and the Social Security Number fields, kindly note that it's not allowed to ask for uploads of sensitive documents, but you can ask for this infor through a short text entry field; however, this requires some mandatory fields, here are the details:

Please be noted that in your form https://www.jotform.com/build/82575037659165 you have 195 submissions so once we delete the field it would also delete the submissions related to the SSN and DL fields.
