Why Account Suspended?

My login is Topsystec. I´d link to know what happen 'cause my Account is suspended. 

Our apologies for the inconvenience. Your account was suspended because you are collecting ID Numbers on your forms, and  Free accounts are only allowed to collect ID numbers.

If you wish to collect Sensitive information such as ID Number, you must add the two mandatory widgets 

1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

2. Add a Signature Widget so your users can sign and agree to provide such sensitive information. 

However, before reactivating the account, kindly authorize us to remove the ID numbers field form your form.
