How to enable people who are filling the form to add multiple rows?

We also need to add in a table with multiple rows or the ability for someone filling out the form to add in multiple rows. At the moment our tables only have one row unless we add a title for both the rows and columns which we do not want to do. Please can you advise us on the best way to get the form looking and working as shown on the attached screenshots?

If you want your users who fill the form to add multiple rows, you can use the Configurable List widget in the form.
