    I was trying to expedite our new employee's paperwork (which asked for a picture of their ID and SS card).  I think this triggered the system to think I was phishing.  I can take that form down and have them do it in person.  Let me know what else I need to do to get my forms back :).

    Your account was suspended, because you collect file uploads of passports, driver's licenses and SSNs. Please note that this is not allowed and it violates our TOS. You can read it here:

    If you do not collect this data from the general public, but from your own clients, you can collect passport, driver's licenses and SSN numbers. You need to comply with two things:

    1. You should add the Terms & Conditions widget (or the Short Scrollable Terms widget) for your clients to read and agree.

    2. You should add a Signature widget. So, your clients can sign and agree to provide such sensitive information.

    Please have in mind that it is only allowed to ask for passport, driver's license and social security numbers. You can not ask for any kind of file uploads for these documents.

    If you want to reactivate your account, we would need to remove the offending fields. Then if you want, you can adjust your forms and collect only numbers and add the above widgets.

    Please tell us how to proceed.

    I have delete the offending fields and reactivated your account. So, you can now login again and use your forms.
