Replicating a field's contents further down a form

Hi folks, 

An online form I use is generated in PDF form for electronic storage. It runs over 2 pages.

I need the second page to have a header, or similar, that contains identifying information about the patient, taken from fields at the top of the first page.

How can I set this up?

Hello Chris - It's possible to pull data entered in form fields using a text field. Simply get the form fields unique name and put it in the text field, please check my screencast below for reference.
