Create form for patients

    I would like to know if we could use your software to make the same form within the google environment ( we can import the data into excel file and run some analysis). Our form is arranged as tables, whereby, the patient can check the boxes never, sometimes, often and fill in some numbers and answers in words. Google form is not satisfactory to us because it can't do table layout, and it will take about 150 pages if we use it. Also, I would like to know how to distribute the form to our patients and return to us securely? Do you have any suggestions?

    You are able to use the Input table element where you are able to create a table with different input elements.

    Additionally, you are able to set up Google Sheets integration, which will send all submissions to your Google Sheets, if you need it to.

    Each form has a unique URL that you can send to your patients to fill them. Additionally, you can email the form to your patience from Jotform directly. All you need to do is to enter their emails. Please note that you have a limit of 5 emails per message.

    After they fill the form, you will get submitted data via email, and you will be able to check all submissions from your Jotform account directly.

    Jotform is using 256 bit SSL (Secure Socket Layer) connection that uses a SHA256 Certificate, so all the data is securely stored on our servers. 

    For additional protection, you can enable submission encryption, which will add an extra layer of security.
