Can audio be added to the image & video section.

To clarify, you would like to upload an audio to your form on the same page where the video is located, correct? 

I checked your form and it seems that you are using the Card Layout (single question in one page). Unfortunately, it's not possible to add multiple fields to this layout. You can switch to the Classic form layout to meet your requirements: How-to-Change-the-Form-Layout
