    My account is blocked. Can you let me log into my account please ?

    Thank you for contacting us.

    After checking your account I can confirm that was automatically suspended, I already activate the account.

    If you would like to gather sensitive information, you will need to do the following:


    1) Insert a Terms & Conditions widget into your form with your Terms of use

    2) Insert a Signature widget into your form so your respondents can acknowledge that they are providing sensitive information

    3) Change the email address on your profile to a corporate one.

    4) Send an email to reviewers@jotform.com with proof of your affiliation to a legitimate business and request a review of your account.
