I get an error message that my form did not pass the status check? Any ideas?

    For some reason, our system autosuspended your account. I have removed the suspension. Please confirm that everything's okay now.

    Incidentally, the form at the link you provided is in your Trash folder. If you go into that folder, click the form and then click the Restore button, like the example below, it will be active again:
