Why is my account suspended?

    Hello,


    My name is Tal and I've been the chief architect and CTO of several companies for the past decade. As part of my job I conduct POC's. Some of them are related to dynamic forms. As such I had some samples I built to demo css flexibility, etc.


    For some reason my account has been suspended. I will stress that none of my forms has ever gone public.


    Please advise,

    I did check your account and found that it is currently suspended because of the copyright logo you have added in your form " Payoneer Sign Up". Please check the screenshot of the logo:

    Are you authorized and have permission to use this copyright item in forms? Please send us proof that you are authorized to use this logo. 

    Since your Jotform account profile does not show an email address related to it, this is violation of our Terms of Use. Please change the account profile email address to one that matches the domain with logo, or in order to reactivate the account, we should remove the logos on your forms.

    We will wait for your response.

    Thanks for the prompt reply. I am indeed the director of architecture of Payoneer. Some of my POC's are done on behalf of a personal account, unrelated to my job. You can view my profile here:

    It could have been simpler had you let me know and ask me to fix it. As my login is locked I am unable to do that. If you wish to remove the logo go ahead and do so.

    If you are authorized to use the logo, you need to change your account profile email address to one that matches the domain with logo.

    For now, I have deleted the logo image from your form and activated your account. Do try to sign in to your Jotform account and get back to us if you have any trouble.
