    I cam across your article on setting up a form to tell where a user comes from when they fill out a form. However, I need instruction on how to do this if I'm doing a Pop Out. Is this possible?

    Would also like instructions on how to do it in a light box! :)

    Thank you for any help!

    Hi,

    You may want to check this other thread in our support form I-would-like-to-know-the-URL-where-the-form-was-used-using-a-Lightbox-form

    In my understanding the requirement was similar to yours.

    This was a part of my response on that thread

    You may want to check Adding-parameters-in-Lightbox-and-Feedback-formsarticle in JotFrom guide to understand how to pass value in a lightbox form.

    I made a test page using the lightbox form here. You can also check the test notification email here.

    This the lightbox code I used

    <script src="//www.jotform.me/min/g=feedback" type="text/javascript">

    new JotformFeedback({

    formId:'22246053313442',

    base:'http://www.jotform.me/',

    iframeParameters: {

         'test1':  document.URL},

    windowTitle:'Clone of Clone of Contactformulier',

    background:'#FFA500',

    fontColor:'#FFFFFF',

    type:2,

    height:500,

    width:700

    });

    </script>

    <a class="lightbox-22246053313442" style="cursor:pointer;color:blue;text-decoration:underline;">Clone of Clone of Contactformulier</a>
