My 'getting started' form is not working
I have embedded, + html snippit

That is because of the z-index property of the image borders around the form. I suggest you to removed that so your form will work.
Find this line:
<div id="id2" style="height: 770px; left: 83px; position: absolute; top: 461px; width: 510px; z-index: 1; " class="style_SkipStroke_1 shape-with-text stroke_0">
and remove: z-index: 1;
