Mail entire form submission as one entry

Good day,
I have 7 pages created, and would like to name them according to subject for conditional statements. How do I change the name from "Page 1" to a custom name?

Unfortunately, you can not give names to the pages in the Form Builder.
The pages will appear in the conditions as "Page 1", "Page 2" etc. In order to setup your conditions, you will need to find the number of the page and then setup the condition with this number.

Thanks ... got that part figured out.
I tried submitting the form and it mails separate pages. How do I get it to mail the entire form as one entry?

To clarify, when you receive the notification email it is split to different pages?
I have tested a cloned version of your form and I was able to receive the notification email as one entry.
Kindly try to create a new notification email and let us know how it goes.
