How do I add fillable PDF forms to my website and submit from website?

    We have a medical website and trying to make all of our intake and consent forms fillable online from our site. I had someone turn all of our forms into fillable PDFs but not sure how to get them onto our website and then have patients be able to submit them from the website to a main email. Can your company help with this?
