Please try to clear your form's cache and try to relaunch. A guide on how to do that can be seen here: http://www.jotform.com/help/229-How-to-Clear-your-Form-Cache
If you're still not able to see the form, you can also try clearing your browser's cache

Well I tried for over an hour and got nothing. But I'm glad it's fixed now.
