    Hi Stephanie,

    Your forms seems to be working fine on my end upon checking. Have you tried already clearing your browser cache and cookies? If not, can you please give a try?

     in my computer and the problem continues to empty the cache and cookies?

    The form is working fine in my end too, as was told please follow this steps to clear your browser's cache this must be the problem.

    You will also want to clear the forms cache.

    Please replace your embed code with the correct form URL. This should fixed it.

    FYI, we already have assigned different domains for every users depending on their profile location. It seems on your location, you are required to use form.jotformz.com. You can use the following iframe embed code with this correction.
