We were considering your software for our organization but had a pre-sales question. The forum required us to register with our paypal account email which we cannot do without first getting more information. I will ask our question in the hopes that it might get answered. Can your software build forms with dynamic fields (best way I can describe it). You can see on the form in the sidebar on the site below, there is an area where you can add a spouse and add children. Can your system do this? http://healthplansofnc.com/

To register an account with Jotform, it is not necessarily needed that you use your PayPal email address, you can alternatively use your personal or company email address for your account registration. Regarding the example you have provided, that is possible with Jotform form builder. You can use conditions to hide and display field.

I have created a form which can be found on this link that when you select 'Yes, I have' it will display a full name text field.

If you have any other question you would like to ask, please feel free to contact us again.
