We are very sorry about that. A recent changed caused your JotForm Hosted account to stop working. Since it was setup very long time ago its setup was different from other hosted accounts and this caused us to not recognize the problem.
I have fixed it now. I will provide refunds for the down time. I will also setup monitoring for this account.
