When editing my form I chose "YES" as the selected radio button but the live form always shows "NO" as the default.
Oh geesh! I ALWAYS try that first...well, until this time :)

When I went back to the actual site and cleared my cache it still shows up as "NO" on my end.
What do you see?
I've check the link of your website that you provide but I couldn't get in cause it requires some WP login information.
Have you tried already re-embedding your form code into your website using iFrame method? If not, can you please give a try?
