This random Quantity" menu is ghosting around on my layout. What gives?

I cannot see the error you are having when I go to your form "Student Information".
Kindly clear your browser cache and cookies and check back again and check if you will still have the error.
If problem persists kindly let us know so that we could further investigate.

It appears in the JotForm editor. It comes and goes randomly, sometimes not even showing completely.
Yes, of course I cleared the cache (I seem to need clear the cache about every 2 minutes when working with JF) but the problem persists.
Newest releases of Chrome & MacOS X.

I cloned your form and was able to encounter that particular issue as well.
It's a good thing it is aligned properly in the live form but this is something that we will still have to refer to our developers. Thank you for reporting this issue.
