After you open "Submissions" page of a form of yours you can see a wheel button on the right hand side. When you click on it you can see some options you can set and some fields you can include to your submissions. When you click on "IP" field you can include IP address of submitter in each submission.

There are two places where you can see the form submitter's IP address. You can have the IP displayed either on the form's Email Alerts or on your Submissions Page.

To show IP address in Email Alerts:

    1.  Go to Setup & Embed tab

    2.  Click Email Alerts button in the toolbar

    3.  Click the Email alert where you want the IP address to show

    4.  Insert the characters "{ip}" (without the quotes) anywhere in your email template

    5.  Click finish and save your form

To show IP address on your submissions page:

    1.  Go to My Forms page

    2.  Click on the particular form then click the Submissions button on the toolbar

    3.  While in the submissions page for that specific form, click the small settings icon found on the right side of the page, under the Delete button

    4.  Check the "IP" option from the context menu that pops up
