     I would suggest to re-embed your form using iframe embed code. This should help fixed this issue. Getting the Form iFrame Code

    The caused of the misalignment is probably because of the width is not equalized for each field. Browser tends to work differently in accepting width:auto element causing this to happen. Please try adding the CSS code below in your form styles to add a fixed width for each form line column and see if it resolves the issue.

    .form-line-column { width:400px; }

    .form-line-column { width:400px !important; }

    To add the CSS code in your form go to Setup and Embed> Preferences Icon then click the Form styles tab and add the CSS code in the text area.

    If the issue still persist, please contact us again.
