I checked all 4 notification email alerts on your account and searched our mail logs.
Both the gmail and carstarmundelein.com email logs indicates that the notifications
were sent however the 2 aol addresses were queued.

Please add JotForm to your whitelist addresses to avoid being tagged as spam.
Kindly also check the bulk, spam or junk folders in your inbox.
There are instances that emails from jotform are caught by the spam filters.
