Can you please try to apply the solution provided by my colleague just for the notification email which directs to your mail site email?
I have made a test on your form and based on our mail log the notification email was successfully sent to: firstname.lastname@example.orgCurrent Server: /var/log/jotform/amazonSES.log:[14/Jun/2013:12:04:16 -0400] 31368364940458 AmazonSES Sent email to email@example.com - MessageID:0000013f436e1a03-0190cf3a-5745-4b37-947c-966dd164ab02-000000 - RequestId:1051dc0a-d50c-11e2-81dc-95e1b13794d0

I checked your Request Song Form and it has this email as the recipient: email@example.com
As you notice, it has .info in the domain. However, according to your post it should be .com. firstname.lastname@example.org
I would suggest to probe it further. Thanks!

Got it, thanks for the clarification.
By the way, I just tested your form now. I have submitted a test submission to your form and according to the mail logs, the message was successfully delivered to email@example.com. I would suggest to review it again on your side. Check your inbox & spam folder.

Also, if you continue to have this issue with this specific email address, I would suggest to whitelist the domains that are used by one of our email service provider (Amazon SES) to your email provider side. Check this article: http://www.jotform.com/help/208-How-to-setup-email-alerts-to-prevent-emails-not-being-received-issues. See the suggested domains for whitelisting.
