    I need to create the following forms using JOTFORM, are you likely to revoke my account due to the password field?

    Hi Kishor,

    Unfortunately our password field can only be used for our conditional logic tool/multipage forms and the forms that you have displayed above all have password fields and do not fill the above conditions.

    We will definately revoke the account of the forms, Thanks for understanding.

    Could you please recommend a solution as to how I can achieve the same result to transfer this forms to JOTFORM. Please point to the guide for conditional logic tool / multipage forms.

    Hi, Sorry! All the sample Forms that you have provided contain username ans password field. According to our terms and conditions, a form should not contain the username and password field.

    conditional logic tool / multipage forms is not a solution for your problem.
