PDF Converter: not receiving the original PDF after form submission, but default PDF document

I created my forms by uploading a PDF form that I use in the office, and then populating it accordingly through Jotform's platform. When I receive the PDF attachment with the email, I am prompted to input a password which is great. However, the attachment that I open does not look like the form that I use - it has the information, but the format is not the same. I would like this PDF attachment to reflect the form that I am using, because changing the format of the final document creates added difficulty. Is there any way for the PDF that is sent in the email notification to reflect what my actual form looks like?

This is a re-post of a comment on How to Include Submissions as PDF Attachment in the Email Notifications

Are you referring to the form "201197082042043 - Release of Information Authorization Form"?

If yes, I can see that you have enabled the original PDF to be attached to the email:

I cloned and tested your form and I did not receive the original PDF as set in the form, but got the default PDF from JotForm.

Could you confirm if this is the form, so that we could test further and if required, escalate.

Yes this is the form. I also have had the same problem with my other forms.. I would like for the original PDF to come attached as a password-protected attachment in the email, but it only shows the default Jotform PDF

I have cloned your form as well and tested it and I was able to recreate the issue.

Hello, the issue is fixed now. You should be able to see the importedPDF attachment on your emails, with the password set.

If issue persists, please reset your pdf's password and set it again.
