Thank you for sharing your concern and we apologize for the inconvenience. It seems that you're using a guest account to create your forms. You must associate this guest account with a free JotForm account by registering to JotForm.
You should see something like this on the left side when you go to JotForm website:
Just create an account by clicking the button and use your own email address. Your guest account will be automatically merged with this newly created account.

I have checked your email address and indeed, I found five forms that uses your email address, although what you have is a guest account. You do not have a personal login account.
What you should do is create a free account by clicking the orange button at the lower part of the username and password sign-in fields.
Please make sure that you use the same email address that you have in your forms so that it will automatically be merged to your new free account.
