The required fields are completed however when I receive the forms some fields are blank. "Clients Name & Neutered/Spay" field questions.

That's because your email notification failed to update when you make changes with your form. For the missing data, you can check that in your form submissions page.
In order to fixed the blank problem on email notification, simple recreate it. Delete your existing Email Alert Notification and create a new one. Here's a guide:
