    Hi Lee,

    Judging by the error message it looks like you have tried to sign up for an account twice with the same email address.  If you are trying to retrieve your account details, go to the My Forms page (http://www.jotform.com/myforms/) click on the Forgot Password button at the bottom of the page and enter your email address to reset your password.  I've checked our email log and there was an email sent on the 24th to the address you've given, so the system knows you and resetting will solve the problem.
