Hi, Yes! You can create such forms very easily using our JotForm Form builder. Since the Form you provided is not related to JotForm, It is not possible to import that form to JotForm. But you can Create by your own in 10 mins using JotForm.

You can use Copy/Paste Option so you won't type them again when creating/adding the fields in our form builder. You can easily create them as we have all the fields existing in our form builder.
You can use our Matrix Field for the Table with Radio button on the provided example.
