Why newly created forms are being disabled?

    Please advise what features the following form contains that are preventing me from publishing.

    I cloned your form and I was not able to replicate the issue.

    Thanks for the effort, but that does not resolve my concern.

    I'm attempting to create a HIPAA Compliant form for use in my office. A critical part of the form creation process is being able to view the form at different stages of development. I keep running into an error and hope that you are able to effectively advise.


    This is a blank, HIPAA compliant form. The form is enabled in My Forms page. Prior to any changes, the form is unavailable for preview, eliciting the "Form not found This form is disabled" error on the Preview Form page. The form is disabled for all platforms and orientation combinations.

    Would you please advise why a brand new, blank form is disabled? I hope that your guidance will help me in resolving the unavailability of a form that is currently in progress but impeded by the same error.

    It's possible that your forms are on HIPAA server, but your account is set to Free. Please note that HIPAA account can only be enabled on a Silver or Gold plan. You are not able to view these forms since they are on HIPAA server and your account went to a regular account viewing on a standard server (not HIPAA server).

    Your options are, you will need to upgrade to Silver again or switch your account back to non HIPAA account.
