Why has my account been suspended?

    I don't understand why my account was suspended

    Could you please provide us with your account username, registered email address or even a URL of any form associated to it, so that we can better assist you?

    We apologize for inconvenience. I checked the account/username Scaffoldstaffing  and I noted that it was suspended by the Phishing system due to the suspicious form that violate the Terms of Use.

    If your purpose is not to collect these data from the general public, but from your own customers, then we can make an exemption provided that you comply with the following requirements:

    1. Add Terms of Use widget (for your users to read and agree). This will surely make your forms not to be considered as guilty of phishing activities

    2. Add a Signature widget where your users sign and agree to provide such sensitive information. 

    We can reactivate the account but we need to remove the guilty fields on the form first and ensure that you agree with our Terms.

    I was in the process of adding signature fields and everything when it was
    suspended. I uploaded the form from the irs website but everything didn’t
    convert properly so I was doing it manually

    I was trying to delete that form that was offensive and it suspended my
    account again. Can you delete the form requesting the tax ID Number and
    restore my account so that I can continue working. Also can you make sure
    my other forms remain active. It took a long time to create them.
