I have another question related to another form that I created similar to the Card form but using the normal form.
I would like to set the condition and prompt the same message as in the Card but I can't do it. Here is the link to the form: https://form.jotform.com/201410129078446.
What I did was I created another page to prompt the message but after submitting the form, the message on another page will be in the pdf form. As shown below:

On your card form, when you disable the submit button, they will not be able to submit.
As such, I would suggest that in this form, you use the Hide condition to hide the submit button, whenever they show any possible sign of COVID.

Please also make sure to only have one condition that hides the submit button to avoid any conflicting conditions.

Hi Vanessa,
Correct by disabling the submit button, they will not be able to submit the form. But is there any way that I prompt a message to inform that there are not allowed to enter. Because by disable the submit button, some people might not understand the reason behind it.
As for setting the condition, I will make it into 1 as to avoid conflicting conditions.Thanks!
