How do I use privacy policy and terms and conditons on form

    I am using a template called camden chiropractic history form. I have made some minor alterations.. I just want to see how the whole thing works. Once I have created the form it is not possible to fill it in because it keeps coming back with the message:

    Incomplete Values
    There are incomplete fields in your submission: Terms & Conditions is required Privacy Policy is required Please go back and fix the problem(s)
     
    I do tick the box but each time I go back it has unticked itself.
    I have set up the link on the widget to the privacy policy on my website... What am I doing wrong???

    Hi! We have two layouts, one is Simple layout and the other is Card layout. Looks like you imported a Card layout Form and then changed the layout to s Simple Form.

    Not all the Form elements are work for both layouts; so if a shift has been made, unsupported Form elements need to be removed.

    Following is what you can do to make your Form work:
