Create a form for "order inquiry" from 100+ list in imported spreadsheet

Is there a way to import or copy/paste "BOOK NAME"   and then add a pull down for each re: QTY to make my form. I only see a template where each product is entered individually.

Please be careful to match the headers in the CSV our Excel file so that all data will be imported.
