How to change the form title when shared?

Hi, I have two different forms. One is called Canine Country and another is simply ASV. Now, when I share my ASV form it comes up with the canine country in the wording. Why and how do I get rid of it? 

I have checked your form and it seems you have not updated your page title.

Kindly change the Page Title  in your Settings>Form Settings > Page Title
