How can i change the jetform to read the correct title?

Hi,

I have 2 folders that were previously named HME SHUTDOWNS however i have renamed them to Rostered Maintenance. But when i go to the link to start the review/evaluation it still comes up with HME SHUTDOWNS as the title?

Any information on how i can change this would be much appreciated.
