I may be confused, I wanted to set up so when the form is submitted, we recieved and email and the person submitting the form also reseived one. I tested the form and I recieved an email the info was added to our Google docs but the email address I put in the form did not recieve an email.
Is it possible to send the form for thier records? or did I misunderstand this part.

I checked your form's auto responder setup and dont see any issue with that. I checked our email server logs and the email was not delivered. Most probably the reason for this email message were not delivered is due to some problems with AOL's mail server:
/var/log/mail.log:Jun 28 14:22:21 us-app2 sm-mta: r5SH7x1O031861: to=<email@example.com>, delay=01:14:22, xdelay=00:00:00, mailer=esmtp, pri=663786, relay=mailin-03.mx.aol.com., dsn=4.0.0, stat=Deferred
Request you to please check your auto responder with a non AOL email id and see if you continue to have this issue.
