how do you add a "remember me" checkbox to a section of a form only

    I have a form that doctors can fill out to request an appointment or referral.

    The first part of the form will be the same each time the same GP (from the same computer in their office) opens the form.

    At the moment, they said that this is redundant (such as their provider number, address, practice name and fax number) because its the same for the first time they do it.  The rest of the form (patent details) will of course be different each time.  The feedback I have got is that this is the reason why they won't use it.

    Any ideas on how each to achieve this?

    Hi, Yes You can acheive this with some workarounds.

    You need to create 2 forms, one for collecting Doctor's detail and one for feedback.

    In form 1:

    Add a unique Field to the form.

    In Form 2:

    Add a hidden field to collect Doctor's unique code.

    Now change the thank you URL of Form 1 to point to Form2 with Unique ID prepopulation. So that Doctor's need to allways use their link to fill out the feedback, Instead of filling their details again and again. 

    In Submissions you can easily compare the Unique Codes to relate 2 forms.
