Account Suspended. Please help me.

    Hello,


    I'm not sure why my account was suspended. Please look into the matter and help me at the earliest as my website operations came to a sudden stop. If there are any changes to be made, please let me know as well.

    Hello Sonal,

    Your form 201143375997058 has fields collecting Social Security Number, which is against our terms of use. We can delete this field for you if you let us do so. Then we can activate your account.

    I have deleted the offending field and reactivated your account.

    Please note, that if you are collecting sensitive information you should add two mandatory widgets to your form:

    1. Terms and Conditions Widget - detailing your terms of handling the sensitive information

    2. Signature Widget - where the user agrees to provide this sensitive information

    My colleague removed only the "Social Security Number" field from your PATIENT INFORMATION form
