Please keep in mind that JotForm is a form builder. We do not collect payments. Our forms are used by millions of users for collecting surveys, registrations, voting, polls, orders, subscriptions, donations, etc.

Upon checking the form, there's a note that there's no refund. 

I suggest contacting the National League for Nursing for more information.
